General Description Meets clinical requirements of neonatal and pediatric ARCP care team member. Plans, directs and participates in the delivery of patient care. Coordinates the plan of care for patients with complex respiratory disorders from acute care to outpatient management. Acts as a liaison between the patient and healthcare personnel to ensure necessary care and equipment is provided promptly and effectively. Responsibilities include identifying and educating on health risks, monitoring patient compliance with current disease specific clinical guidelines, coordinating with outpatient clinics, educating patients and their families on their specific disease process, and providing ongoing assessment, monitoring and re-evaluation of needs. Provides support to inpatient RCS team with education and policy review with evidence based standards of care.
Associated Outcomes The RC Transition Navigator works independently utilizing established protocols and critical thinking skills to act as a positive change agent to improve the client's health status; works collaboratively with primary care providers, specialists and the health care team to provide case management services to patients with chronic respiratory support needs. Outcomes may include pressure injury, days to achieve home care regimen, days to care area transition, days to discharge, readmission, access to home equipment, and cost reduction in addition to other outcomes identified by the interdisciplinary team or leadership. Targeted Patient Populations Tracheostomy, home ventilator dependent, non-invasive and pulmonary clearance patients through Duke Children's.

Work collaboratively with physicians, nursing, and other members of the multi-disciplinary team providing targeted management and education to patients diagnosed with a complex repiratory disorder. Develops individual action plans that help coordinate care, manage costs, and open lines of communication.
Navigate the patients care to include contacting appropriate team members/agencies to assist in the determination of patients' needs and/or eligibility for services.
Monitors procedural/clinic schedules to ensure appropriate needs and follow up calls are initiated and patients are aware of what they should expect for inpatient or discharge processes.
Participates in quality initiatives to drive improved patient outcomes.
Helps ensure service growth by participation in outreach strategies such as health fairs, screenings, and community education.
Completes projects in assigned timeline. Analyzes project data for departmental process improvement. Demonstrates ability to prioritize and efficiently manage assignments.
Demonstrates ability to communicate effectively with patients, families, co-workers, and physicians. Quickly adopts changes needed to meet customer needs. Delivers care in a nonjudgement/nondiscriminatory manner that is sensitive to linguistic, cultural, and spiritual needs. Displays conduct that fosters the growth and development of a professional and harmonious work environment.
Appropriately documents encounters with patients as well as submit billing documentation and coding in a timely manner to ensure proper reimbursement of services provided.
Serves as a point of contact for referring physicians, patients, and caregivers to provide assistance with accessing clinical and supportive care services for their disease process such as pulmonary function testing, pulmonary rehab referrals, family smoking cessation, sleep studies, and community resources and support groups.
Routine patient care will be provided by the assigned RCP unless a specific assessment is required, at which point the Navigator will communicate with the assigned RCP.
Weekly rounding with care team and Navigator to review patient progress with plan of care and communication with assigned RCP following.
Tracheostomy discharge education and device education will be overseen by Navigator. Initial education and Emergency scenarios will be conducted by Navigator or specific team. All other classes can be taught be assigned RCP.
Completion of a 2 year AMA approved respiratory care program requirements (Associates Degree or the equivalent). Must maintain annual skill maintenance requirements Must maintain annual safety/compliance Must maintain North Carolina annual license requirements for CEUs
Six months to one year of clinical experience following graduation from a respiratory care program.
